You will diagnose, develop individualized treatment plans, and work across Family Medicine, Oncology, OB/GYN, and other specialties to support whole-person care.

Qualifications

Responsibilities

• Provide diagnostic assessments, onsite crisis assessments and interventions as needed.
• Be proficient in multiple counseling modalities and make appropriate referrals.
• Serve as primary team consultant to integrate health and behavioral factors in care.
• Collaborate with PCMH/PCC teams to address psychosocial and cultural health elements.
• Document responsibly to ensure safe, patient-centered care, following ethics.
