Job summary

Adult & Child Health in Indianapolis is seeking an Integrated Health Nurse to deliver comprehensive nursing services across settings, including residential facilities, behavioral health programs, and community visits.

You will perform assessments, coordinate care with the treatment team, administer medications, monitor labs, educate patients on disease prevention, and document promptly in the electronic health record while maintaining confidentiality and compassionate care.

Qualifications

• Current Registered Nurse licensed in the state of practice.

coordinate any required action planning based on results.Monitor the health status of the client/patient during services provided.Work with the treatment team to coordinate care based on client/patient needs.Create action plans for client/patient based on physical and mental health in regard to treatment needs.Oversee infectious disease control, client/patient hygiene, and related client & facility health issues, under the supervision of nursing leadership.Provide disease prevention, risk reduction and other health education as needed.Arrange medical appointments for client/patients dealing with medical conditions that occur during treatment and manage medical emergencies, such as accidental injuries and/or drug reactions.Consult with a physician/advanced practice provider as necessary or advocate for medical services through managed care organizations.Arrange or monitor special dietary needs for medical conditions.Review medication requirements with clients/patients, educating the client and guardian (if necessary) about the benefits of taking prescribed medications as prescribed.Administer and oversee administration of medications via various methods according to service line requirements;
