Job summary

Southcentral Foundation (SCF) is seeking a Community Resource Specialist I, II for the Benteh Nuutah Primary Care location. The role focuses on coordinating Federal, State, and Local programs, services, and resources with clinic staff and customer-owners to improve care and access to resources.

The position supports care coordination tasks across Integrated Care Teams, working with referral sources and providers to enhance the customer experience and overall quality of care.

Qualifications

• BA or BS degree in Nursing, Psychology, Rehabilitation, Social Work or a closely related Human Services field from an accredited college or university; OR equivalent combination of education, training, and experience.
• Master’s degree in Counseling, Education, Marriage Family Therapy, Psychology or Social Work; OR equivalent combination of education, training and experience for Community Resource Specialist II.

Responsibilities

• Coordinate a broad range of Federal, State, and Local programs, services, and resources in collaboration with clinic staff, customer-owners, and social resource sources.
• Perform time-intensive care coordination tasks and work with Integrated Care Teams, referral sources, and provider teams to improve the customer experience and overall quality of care.
