Job summary

Health Connect America is seeking an Extender role to support care coordination for members across the multi-state footprint. The position involves outreach, engagement, and coordination of services under Tailored Care Management, working with care managers and the care team in homes and communities.

Ideal candidates will have a Bachelor’s degree (human services preferred) and be at least 18 years old, with lived experience or peer specialist credentials as described.

Qualifications

• Bachelor’s Degree in any discipline is required; Bachelor’s in human services is preferred.
• Be at least 18 years of age.
• Must meet one of: lived experience with I/DD/TBI and Medicaid navigation knowledge; or be a Certified Peer Support Specialist with lived experience; or a parent/guardian with relevant experience; or two years paid, with one year directly working with Tailored Care Management eligible population.

Responsibilities

• Perform general outreach, engagement, and follow-up with members.
• Coordinate services/appointments (reminders, transportation).
• Engage in Tailored Care Management activities and knowledge sharing.
• Share member information with care manager and care team.
• Provide and track referrals and assist in obtaining community resources.
• Participate in case conferences and support the care manager.
• Document all efforts in ONEcare promptly.
• Attend virtual team meetings and in-person trainings as required.

Our Brands

The Integrated Care Extender supports Integrated Care Coordinators in delivering care coordination and management services through performing varying activities.

• Performing general outreach, engagement, and follow-up with members
• Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation)
• Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing
• Sharing information with the care manager and other members of the care team on the member’s circumstances
• Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services
• Participating in case conferences
• Support the care manager in assessing and addressing unmet health-related resource needs.
• Document all efforts within the care manager platform (ONEcare) timely
• Participate in virtual team meetings and be required to attend in-person training/meetings as determined by leadership

TN

• Bachelor’s Degree in any discipline required. Bachelor’s Degree in human services related discipline preferred.

NC

• Is at least 18 years of age
• Has a high school diploma or equivalent
• Meets one of the below requirements
• Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system
• Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist.
• A parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member)
• Has two years of paid experience performing the types of functions described in the “Extender Functions”, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
