Job summary

Essen Health Care is seeking a Licensed Master Social Worker to perform daily social work operations within a multispecialty medical group in New York. The LMSW will assess patients, provide behavioral health treatment, and collaborate with PCPs, nurses, and care managers.

Ideal candidates hold an MSW and LMSW licensure, can use DSM-V criteria, and are comfortable with care coordination and documenting in EHR systems. Bilingual Spanish is strongly preferred for serving our diverse population.

Qualifications

• MSW degree from an accredited program with LMSW licensure.
• Ability to perform biopsychosocial assessments and use DSM-V criteria.
• Experience coordinating care and documenting services in EHR systems.
• Bilingual Spanish preferred to serve diverse patient populations.

Responsibilities

• Assess patients referred by PCP using evidence-based tools.
• Provide behavioral health treatment using CBT, motivational interviewing, crisis intervention, and IPT.
• Coordinate care with clinical team and health homes; participate in weekly supervision.
• Document services timely and participate in team meetings and case conferences.
