Job summary

Care & Help Home Care, LLC is seeking an Intake Specialist to manage the patient intake process, gather essential information, and ensure accurate enrollment into homecare services in a compliant, timely manner.

Qualifications

  • Proficiency in Microsoft Office applications and experience with HHA Exchange software.
  • Understanding of home care services, insurance verification processes, and enrollment procedures.

Responsibilities

  • Handle the initial intake process for new patients, including gathering essential information (e.g., medical history, demographics, insurance details).
  • Ensure accurate entry of patient information into the company’s EMR system and other relevant databases.
  • Verify clients' insurance coverage and eligibility for home care services, ensuring accurate billing and reimbursement.
  • Answer incoming calls from clients, caregivers, and healthcare professionals, addressing inquiries and providing information about available services.
  • Conduct comprehensive initial assessments to evaluate clients' healthcare needs and determine the appropriate level of care required.
  • Collaborate with marketing representatives to stay informed of updates and changes, providing quick and relevant information to clients and stakeholders.
  • Assist clients in accessing CHHA, MLTC, and PCA services.
  • Educate clients on the eligibility criteria for home care services, explaining their options and helping them make informed decisions about their care.
  • Serve as the primary point of contact for patients, families, and referral sources to answer questions and provide updates regarding the intake process.
  • Maintain confidentiality and ensure compliance with HIPAA and other patient privacy laws.
  • Escalate complex cases to the Intake Supervisor or Director of Intake for further resolution.

Job Summary

The Intake Specialistis responsible formanaging thepatientintake process, ensuring all necessary documentation, referrals, and eligibility requirements are gathered efficiently and accurately. Essential Duties and Responsibilities:

ABOUT CARE AND HELP HOME CARE, LLC CARE AND HELP HOME CARE, LLC (“Company”) provides non-skilled services to individuals in their homes or other independent living environments. These services include (but are not limited to) assistance with self-administration of medications, personal care such as assistance with personal hygiene, dressing, and feeding, homemaking such as assistance with household tasks housekeeping, shopping, meal planning and preparation, and transportation, companionship, respite care such as assistance and support provided to the family; and other non-skilled services. We conduct criminal background checks when required on all staff. We also conduct physicals and tuberculosis screenings for all direct care workers and office staff with direct consumer contact upon hiring and on a yearly basis. The direct care workers of the Company are required to complete a minimum of forty (40) hours of experience or practical training and a competency evaluation, or a state certification program. This training includes information regarding personal hygiene, safe transfer techniques, infection control, and basic nutrition. All direct care workers of our company also receive supervised practical training before having direct contact with a consumer or verification of prior experience.
