Job summary

Elwyn is seeking an Intake Specialist to manage day-to-day operations of Mobile Crisis, including intake, triage, admissions, and referrals. You will support dispatch of mobile crisis teams on first shift, collect demographic data, and coordinate with external partners to ensure appropriate care.

Qualifications

  • Bachelor's degree in a human service field; Master's preferred.
  • 3 years of mental health experience with adults, children or adolescents.
  • Non-profit human service experience preferred.
  • Ability to work in a fast-paced environment and prioritize tasks.
  • Must have CPR and crisis intervention training.

Responsibilities

  • Answer and triage calls on the crisis hotline and create admissions in the EHR.
  • Collect demographic information to ensure accurate data and monitoring.
  • Dispatch mobile crisis teams on first shift and support crisis response.
  • Conduct initial assessments to gather admission information from consumers.
  • Track referrals and coordinate with Business Development on referrals.
  • Assist with referrals to alternative levels of care and intake appointments.
  • Assist with entitlements and emergency care provider intake.
  • Advocate with external agencies (SSA, DPW, local health offices).
  • Attend staff meetings and trainings; support self-determination in planning.

Responsibilities

The Intake Specialist is responsible for managing day-to-day business operations of Mobile Crisis including the intake process, including assessment and triage of initial crisis calls. They will assist in obtaining necessary information to dispatch mobile crisis teams on first shift. The intake specialist will conduct admissions, track referrals and maintain relationships with external stakeholders. ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Intake coordinator will answer and respond to all calls that come into the Delaware County Crisis Connections Team hotline, triage calls and obtain all necessary information to dispatch the team, and create an admission to the EHR.
  • Will collect relevant demographic information from all calls received through the crisis line to ensure accurate data collection and monitoring.
  • Dispatch mobile crisis teams into the community from calls received on first shift, to assist the community with mental health support and services.
  • Conduct initial assessments of consumers and their representatives to gather required admission information.
  • Complete tracking of referrals through activities including but not limited to completing necessary spreadsheets and tracking information to ensure proper follow up with referral sources and work with the Business Development team on information sharing about referrals.
  • Assist community members who have been seen by the mobile crisis team with referrals to alternative levels of care and assist with securing intake appointments.
  • Complete referrals for follow-up services by community mental health and other supports. Assist in completion of application for entitlements. Complete Emergency Care Provider Intake.
  • Advocate for individuals served with outside agencies including, but not limited to Social Security, Department of Public Welfare, Delaware County Office of Mental Health, Delaware County Base Service Units, etc.
  • Attend scheduled staff team meetings and appropriate in-service training as directed by Director.
  • Collaborate with the team to promote a team culture in which each person's point of view, experiences, and preferences arc recognized, understood, and respected, and in which the person' self-determination and decision making in treatment planning are maximized and supported increase awareness of and support participation in, self-help programs and advocacy organizations that promote recovery.
  • Perform cardiopulmonary resuscitation (CPR), crisis intervention
  • Maintains current state-mandated training and certifications
  • Follows all current policies and procedures of Elwyn PA and DE
  • Perform other duties as assigned
QUALIFICATIONS
  • Minimum of a Bachelor's degree in a human service field, Masters preferred.
  • 3 years of mental health experience provided to adults, children, and/or adolescents required.
  • Non-profit human service experience highly preferred
  • Demonstrated ability to work effectively as part of a team
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Must be able to perform CPR and crisis intervention using agency-trained protocols
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint
  • (Experience using/knowledge of) Electronic Health Record (EHR)
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
