Overview

We are hiring for a Patient Care Manager Assistant (LPN) with LHC Group. This role supports the Patient Care Manager in coordinating a team to assure high-quality home health care for patients in the assigned area.

Responsibilities

• Ensure the team provides quality care in an efficient manner in accordance with physician orders and State/Medicare guidelines.
• Prepare patient lists and necessary documents for weekly case conferences.
• Provide direct patient care as needed, in accordance with scope of practice, plan of care, and physician orders, in coordination with the health care team. May not alter the plan of care.
• Assist the Patient Care Manager in clinician documentation reviews/audits to ensure complete, appropriate, and accurate submission of all documentation by field staff.

Education and Experience

License Requirements

• Current CPR certification required.
• Current LPN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
