Job summary

Enhabit Home Health & Hospice seeks a dedicated Intake/Referral Coordinator to manage patient referrals and serve as a key liaison between referral sources, branches, and sales teams. You will ensure smooth transition to services while upholding Medicare guidelines and regulatory standards.

Qualified candidates have a high school diploma or equivalent, 2+ years in health care (1+ year in home health or pediatrics), strong customer service, and basic tech skills with mobile devices.

Qualifications

• Must possess a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

Education and experience, preferred

• A graduate of an approved school of professional nursing and licensed in the state in which they currently practice is preferred; may be a registered nurse or licensed professional/vocational nurse.
• Experience with intake responsibilities and working with a multi-location company is preferred.

Requirements*

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions* For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license.
