Job summary

Haven Hospice in Orange Park, FL is seeking a RN Clinical Supervisor to oversee hospice staff, coordinate patient care, and guide clinical decisions in a compassionate end‑of‑life care setting.

This role emphasizes strong communication, leadership, and collaboration within an interdisciplinary team to ensure high-quality care and adherence to standards.

Qualifications

• Must be a Registered Nurse with current, unencumbered licensure.
• BSN preferred.
• Hospice or home care experience preferred.
• Minimum of two years of management or supervisory experience.
• CPR certification maintained per policy.

Responsibilities

• Review new patient referrals and assign appropriate hospice staff.
• Support and guide clinical staff through home visits and case reviews.
• Develop patient care plans and set treatment goals.
• Lead and participate in team case conferences for coordinated care.
• Conduct regular chart audits and provide quality feedback.
• Participate in hiring, onboarding, and training of clinical staff.
• Support staff development and continuing education programs.
• Assist in policy development and uphold professional care standards.
• Engage in community outreach and promote Haven Hospice services.
• Contribute to quality improvement initiatives and organizational goals.

Qualifications

Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation.

Graduate of an accredited school of nursing; BSN preferred.

Previous Experience In Hospice Or Home Care Setting Required.

Minimum of two (2) years of management or supervisory experience preferred.

Current CPR certification, maintained in accordance with organizational policy.

Must be a licensed driver with an automobile that is insured per state and/or organizational requirements and is in good working order.

Proven ability to work effectively within an interdisciplinary team setting.

Demonstrates excellent observation, nursing judgment, and communication skills—both verbal and written.

Understands the hospice philosophy and the emotional, clinical, and ethical considerations surrounding end‑of‑life care.

Knowledge of pain and symptom management principles.

Adheres to accepted professional standards and practices.

Commitment to clinical excellence and thorough documentation.
