Responsibilities

• Creates plan of care using the nursing process: assessment, diagnoses, intervention and evaluation of outcomes.
• Recognizes patient safety concerns and uses appropriate resources.
• Provides education tailored to patient learning needs.
• Integrates patient individuality and shared data in care delivery.
• Performs patient assessments and documents findings.
• Demonstrates teach-back to evaluate patient education effectiveness.
• Uses Evidence-Based Practice in nursing care.

• Documents issues in the plan of care and supports growth for complex cases.
