Job summary

Qualifications

• DPT degree or equivalent
• Active physical therapy license in Arizona or eligibility in AZ
• Experience in inpatient and home health settings preferred

Responsibilities

• Evaluate patients and determine baseline functional status.
• Develop and implement individualized treatment plans.
• Provide therapeutic interventions to restore function, mobility and independence.
• Educate patients and families; document accurately in patient records.
• Collaborate with interdisciplinary rehab team and participate in care conferences.
