Responsibilities

• Evaluate patients and develop individualized therapy plans with physicians and care team.
• Deliver evidence-based interventions to promote healing, independence, and quality of life.
• Monitor patient progress, adjust plans, and document outcomes.
• Educate patients and families to build understanding, confidence, and engagement.
• Collaborate with interdisciplinary partners to ensure safe transitions of care.
