Live Well Homecare Agency, LLC is seeking an experienced inpatient nurse to provide comprehensive nursing care, assess patient needs, develop and implement care plans, and collaborate with interdisciplinary teams to achieve optimal outcomes.

Key responsibilities include assessing, planning, performing and evaluating nursing activities; direct care with treatments and medications; coordinating care and discharge planning; and contributing to quality improvement and safety initiatives.

Qualifications

• Graduate of a specialty nursing program.
• Strong critical thinking, leadership and teamwork.
• Ability to communicate effectively within a healthcare team.

Responsibilities

• Assess, plan, perform, and evaluate nursing activities aligned with patient and family needs.
• Provide direct care including treatments, medications, and diagnostic procedures.
• Coordinate care, educate patients and families, and handle discharge planning.
• Evaluate nursing interventions and adjust plans based on patient responses.
• Collaborate with physicians and staff to address patient needs proactively.
• Participate in unit-based decision-making, quality improvement, safety initiatives, and evidence-based practices.
• Troubleshoot equipment issues and contribute to a team-focused environment.
