Job summary

Peak Rehabilitation Hospital is seeking a dedicated Physical Therapist for a full-time position. You will conduct physical therapy assessments, develop treatment plans and provide direct patient care to restore strength, mobility and function.

You will educate patients and caregivers, document sessions and outcomes, and collaborate with rehab teams on discharge planning. A state license and BLS are required, with opportunities for professional development and comprehensive benefits.

Qualifications

• Degree from an accredited Physical Therapy program.
• Physical Therapist License in State.
• Basic Life Support (BLS) certification within 30 days of hire.

Responsibilities

• Conduct physical therapy assessments and develop treatment plans.
• Provide direct patient care to restore strength, mobility, and function.
• Educate patients and caregivers on home exercise programs.
• Document treatment sessions and patient outcomes accurately.
• Collaborate with rehab teams and participate in discharge planning.
• Perform other duties as assigned.
