Job summary

Clarion Psychiatric Center is seeking a Social Worker to deliver clinical social work services, including psychosocial assessments, individual/group/family therapy, and active treatment planning. You will lead the master treatment plan and coordinate discharge and ongoing care with involved agencies.

The role emphasizes collaborative treatment, goal development, and coordination with external partners to ensure comprehensive patient care.

Qualifications

• Master’s Degree or higher in Social Work or related field.
• LCSW or LSW preferred.
• At least one year of inpatient hospitalization work experience preferred.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Deliver individual, group, and family therapy.
• Actively participate on the treatment team and lead master treatment plan development.
• Develop realistic and measurable short- and long-term goals.
• Coordinate with other agencies for discharge and continuing care plans.
