Job summary

HonorHealth is seeking a Physical Therapist to evaluate, administer, and oversee evidence-based physical therapy services for individuals with disabilities or functional limitations at multiple campuses in the Phoenix area.

The therapist will develop plans of care with the patient/family, communicate with physicians and care teams, and ensure documentation meets regulatory standards. BLS and AZ PT license required; PT degree from an APTA-approved program preferred.

Qualifications

• Bachelor’s in Physical Therapy from an APTA-approved program.
• Master’s or Doctorate in Physical Therapy preferred.
• Basic Life Support (BLS) Certification is required.

Responsibilities

• Evaluates and treats patients using evidence-based physical therapy techniques.
• Develops and updates plan of care with measurable goals and discharge plan.
• Documents evaluations, progress notes, and discharge summaries timely.

Responsibilities

JOB SUMMARY (Physical Therapist)

The Physical Therapist is responsible for the evaluation, administration, and oversight of evidence-based physical therapy services provided to individuals with physical disabilities and/or functional limitations. They develop and update the plan of care in collaboration with the patient/family to assist in returning to their prior level of function, in accordance with physician referral guidelines, regulatory standards, and department policies. They communicate effectively with physicians, co-workers, caregivers, family members, patients, and other healthcare providers.

ESSENTIAL FUNCTIONS

• Implements an individualized treatment plan using skilled, evidence-based therapy techniques (therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, modalities). Provides verbal and written instructions to patients, caregivers, and family members. Maintains current knowledge of PT techniques and regulatory requirements.
• Evaluates patients per department policy. Develops a comprehensive plan of treatment with measurable goals and a defined discharge plan. Performs ongoing evaluation of patient response and documents appropriately.
• Maintains accurate documentation in the patient’s medical record (evaluations, daily notes, updated treatment plans, progress reports, discharge summaries) and completes billing within established timeframes. Demonstrates financial stewardship in resource use.
• Collaborates with the interdisciplinary team to clarify treatment plans, discusses patient status, and communicates discharge needs and equipment requirements. Participates in rounds and maintains effective communication to optimize outcomes. Delegates to therapy assistants as appropriate and provides updated plans and instructions.
• Identifies department needs and participates in process improvements. Supports recruitment, retention, and staff development through professional collaboration.

Education

• Bachelor’s in Physical Therapy from an accredited university approved by the APTA. Required.
• Master’s or Doctorate in Physical Therapy from an accredited university approved by the APTA. Preferred.

Experience

• Other Network float position: six (6) months recent experience preferred.
• 6 months experience preferred.

Licenses and Certifications

• Basic Life Support (BLS) Certification — Required.
• Physical Therapist license (AZ State Board of Physical Therapy) and/or PT Compact License — Current license in good standing; required at hire.
• American Physical Therapy Association (APTA) certificate — Preferred.
