HonorHealth is seeking a Physical Therapist to evaluate, administer, and oversee evidence-based physical therapy services for individuals with disabilities or functional limitations at multiple campuses in the Phoenix area.
The therapist will develop plans of care with the patient/family, communicate with physicians and care teams, and ensure documentation meets regulatory standards. BLS and AZ PT license required; PT degree from an APTA-approved program preferred.

JOB SUMMARY (Physical Therapist)
The Physical Therapist is responsible for the evaluation, administration, and oversight of evidence-based physical therapy services provided to individuals with physical disabilities and/or functional limitations. They develop and update the plan of care in collaboration with the patient/family to assist in returning to their prior level of function, in accordance with physician referral guidelines, regulatory standards, and department policies. They communicate effectively with physicians, co-workers, caregivers, family members, patients, and other healthcare providers.
