LiveWell Homecare Agency is seeking a dedicated Physical Therapist to provide compassionate inpatient care. You will deliver direct therapy, assess patients, and create personalized rehabilitation plans that address physical needs as well as mental and emotional well-being.

Ideal candidates hold current licensure or certification, CPR certification, and a Master’s degree is preferred; a Bachelor’s degree with relevant experience is acceptable.

• Current licensure or certification as per state regulations.
• CPR certification.
• Master’s degree preferred; Bachelor’s Degree with relevant experience acceptable.

Responsibilities

• Delivering direct physical therapy to patients in need of rehabilitation.
• Supervising treatments, conducting patient assessments, and developing personalized care plans.
• Building meaningful relationships by understanding patients’ physical, mental, and emotional needs.
• Supporting patient progress and celebrating milestones.
