Job summary

Ochsner Health is seeking a pharmacist to support medication therapy management within a multidisciplinary pharmaceutical care environment in Louisiana. The role focuses on ensuring safe, effective drug therapy for assigned patients through preparation, dispensing, and verification of medications.

The pharmacist will assess and monitor patient drug therapy, provide drug information, and deliver patient education as part of a comprehensive care team.

• Maintains and ensures complete and appropriate drug therapy within a multidisciplinary pharmaceutical care environment.
• Assesses, evaluates and monitors patient drug therapy; provides drug information and patient education.
