Job summary

Qualifications

• PharmD required or equivalent PharmD training.
• PA Pharmacist License required; PGY1 preferred.
• Current knowledge of hospital pharmacy procedures and policies.

Responsibilities

• Order verification and accurate dispensing of medications.
• Provide pharmacokinetic monitoring and dosing guidance.
• Respond to emergent requests and support dosing decisions.
• Perform IV to PO interchanges where appropriate.
• Review microbiology data and offer drug information.
• Deliver professional, ethical pharmaceutical care to patients and staff.
