Job summary

Carle Health is seeking a licensed Pharmacist to provide pharmaceutical care across Carle Foundation Hospital and affiliated providers in Illinois. The role involves reviewing orders, ensuring safe medication use, and participating in patient care teams.

Qualifications

• Licensed Pharmacist in Illinois (IDFPR).
• Bachelor's Degree in Pharmacy is required.

Responsibilities

• Assist in focused studies and drug usage evaluations as assigned.
• Provide direct patient care by intervening to improve prescribing, administration, and monitoring practices.
• Represent the pharmacy on committees or work groups as assigned.
• Interpret physician medication orders and select the appropriate medication in a timely and accurate manner.
• Screen medication orders for potential interactions and allergies.
• Supervise technicians in preparation and dispensing of medications and parenterals.
• Make dose recommendations based on age, weight and clearance function.
• Assist in training and orientation of new employees.
• Provide accurate accounting of all controlled substances.
• Provide Medication Therapy Management Services as defined in the Illinois Pharmacy Practice Act.
