Job summary

Jobtailor is seeking a licensed pharmacist to provide clinical pharmacy services, optimize drug therapy, and lead medication management for inpatients. You will advise physicians, monitor for ADRs, and ensure medication safety using EHRs and CDS tools.

Responsibilities include reviewing orders, supervising staff, and participating in quality improvement. Active AZ Pharmacist license, BLS, ACLS, and PALS may be required depending on department.

Qualifications

  • Bachelor's Degree in pharmacy from an accredited college of pharmacy.
  • Arizona State Board of Pharmacy license in good standing.
  • BLS certification.
  • ACLS certification.
  • PALS certification for Emergency and Pediatrics departments.
  • 3 months to 1 year of experience preferred.
  • Understanding of computers and basic software programs.
  • Maintain up-to-date certifications and licenses.

Responsibilities

  • Perform clinical services to optimize drug therapy and document interventions.
  • Provide medication therapy management using pharmacokinetic, parenteral nutrition, serum creatinine, and therapeutic drug monitoring protocols.
  • Advise physicians on formulary medications and alternative therapeutic options.
  • Provide clinically relevant drug information to physicians, nurses, and allied health professionals.
  • Participate in code arrest response by providing information and assuring medication availability.
  • Identify, prevent, and investigate adverse drug reactions and medication errors.
  • Provide information for drug-related patient counseling.
  • Review and interpret physician orders; screen for incompatibilities, interactions, duplicates, and contraindications.
  • Contact nursing personnel and prescribers regarding questions on drug orders.
  • Dispense and label oral medications and IV solutions.
  • Use pharmacy technology including EHRs, automated dispensing machines, compounders, unit dosing, tube systems, and clinical decision support/data mining tools.
  • Process patient charges and credits.
  • Assist in maintaining drug supplies from vendors.
  • Exercise legal and professional controls on controlled substances.
  • Verify physical inventory for incoming and outgoing items.
  • Document waste from returned, expired, or unusable controlled substances.
  • Supervise and oversee supportive personnel.
  • Report adverse drug reactions, medication errors, and good catches.
  • Assist in training new pharmacists and pharmacy technicians.
  • Adhere to regulatory, organizational, hospital, departmental, safety, and patient-care standards.
  • Report safety incidents through the Midas/RDE tool and attend safety training.
  • Participate in quality improvement and clinical projects as an active member of the patient care team.

Requirements

  • 3 months to 1 year of experience - Preferred
  • Understanding of computers and competence in using computers and basic software programs
  • Ability to keep all certifications and/or licenses up-to-date and valid
  • Completion of all mandatory modules and required job-specific training within the specified timeframe
