• State of Michigan LMSW or LLMSW license or license eligible.

Responsibilities

• Assess patient/family social and emotional factors to aid adjustment to illness.
• Provide psychosocial counseling and support to patient/family.
• Coordinate with inpatient palliative care team and case management for care planning.
• Participate in interdisciplinary team meetings to establish care goals.
• Document in EMR and ensure timely updates to patient records.
