Job summary

Yale NewHaven Health in Connecticut is seeking an Occupational Therapist to evaluate residents, develop and implement plans of care, and ensure care aligns with state and federal regulations. The role emphasizes clear clinical documentation and multidisciplinary collaboration.

Qualifications

Responsibilities

• Integrate the five elements of patient/client management: Examination, Evaluation, Diagnosis, Prognosis, Intervention.
• Identify tests and measures necessary to guide intervention.
• Implement and modify treatment plans in accordance with patient needs.
• Identify when reassessment or discharge should occur.
• Determine need for intervention through a screen.
• Complete all clinical documentation clearly and in compliance with guidelines.

• 5.5. Will posses the skills necessary to determine need for intervention through a screen.
• 6.6. Will complete all clinical documentation clearly and in compliance with company, state, federal and intermediary guidelines.

Qualifications

EDUCATION

Bachelor of Science (or higher) in Occupational Therapy

EXPERIENCE

6 months preferred in outpatient setting.

LICENSURE

2. Current Occupational Therapy State of Connecticut licensure
