Job summary

MaineHealth is seeking a Medication Reconciliation Technician (MRT) at Maine Medical Center to gather and verify patients’ medication histories at admission. You will document findings in the electronic medical record and collaborate with pharmacists and the care team to improve patient safety during transitions of care.

The role targets detail-oriented pharmacy staff, students or patient service representatives with strong communication and teamwork.

Qualifications

• High school diploma or equivalent is required.
• Active Pharmacy Technician license (or eligible) per state.
• Six months of licensed pharmacy technician experience.

Responsibilities

• Interview patients and caregivers to gather medication histories
• Verify medication information for accuracy and completeness
• Document medication histories in the electronic medical record
• Collaborate with pharmacists and other healthcare professionals to support patient care
• Help reduce medication errors and improve patient safety
