Northeastern Health System in Tahlequah, OK seeks a clinically driven pharmacist to join the inpatient pharmacy team. This role integrates medication verification, therapeutic evaluation, and dosing optimization across oncology, infusion services, and antimicrobial stewardship.

You will collaborate with physicians, nurses, and other clinicians to optimize therapy, improve patient outcomes, and advance safety.

Formación

• PharmD or BS in Pharmacy from an accredited institution.
• Current Oklahoma pharmacist license or eligibility for licensure.
• Previous inpatient or clinical pharmacy experience preferred.
• Strong clinical assessment, communication, and critical-thinking skills.

Responsabilidades

• Inpatient Pharmacy: medication order verification, therapeutic evaluation, dosing optimization, clinical consultation, and medication safety.
• Oncology & Infusion: clinical review of treatment regimens, dosing assessment, supportive care, and safe medication management.
• Anticoagulation Management: patient-specific therapy monitoring, laboratory evaluation, dosing recommendations, clinical follow-up.
• Antimicrobial Stewardship: evidence-based antimicrobial selection, dosing optimization, therapeutic monitoring, collaboration with providers.
• Interdisciplinary Collaboration: team-based care with physicians, nurses, and other professionals.
• Quality & Safety: support medication safety initiatives and continuous improvement.
