Job summary

TriHealth in Cincinnati is seeking an Inpatient Clinical Pharmacist to provide patient-centered pharmaceutical care. You will collaborate with physicians, nurses, and care teams to ensure safe, effective medication use and contribute to clinical decision-making.

Qualifications

• Bachelor's degree in pharmacy is required.
• 1-2 years' pharmacy experience is required.
• Active Pharmacist license in Ohio.
• Working knowledge of Ohio Board of Pharmacy regulations.

Responsibilities

• Ensures the 7 patient rights are respected: right patient, dose, route, time, drug, information and documentation. Reports medication errors and assists in managing risk.
• Leads opportunities for pharmacy process improvements and participates in professional development and committees.
• Contributes to direct patient care with careful review of patient information and provides pharmaceutical recommendations to care teams.
• Precepts pharmacy residents and students and supports orientation and training of new hires.
• Manages drug distribution, inventory, and complete documentation to ensure safe, efficient operations in the department.
