Intermountain Health is seeking a Pharmacist to join the Inpatient Pharmacy Team at St. Marys Regional Hospital in Grand Junction, Colorado. You will collaborate with interdisciplinary care teams to ensure safe, timely and effective medication therapy across the hospital.

The role requires a Doctor of Pharmacy or PharmD with active license, strong regulatory knowledge, and a focus on patient education and safety within a fast-paced hospital environment.

Qualificações

• Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).
• License to practice pharmacy in the state of practice and dispense controlled substances.
• Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.
• Effective communication, people management, and project management skills.
• Immunization certification if applicable to practice site needs.

Responsabilidades

• Reviews patients' medication orders for allergy interactions, duplications, and drug interactions.
• Provides evidence-based interventions to optimize medication use and ensure safe, cost-effective therapies.
• Oversees preparation and delivery of medications to care areas and ensures secure storage and workflow.
• Counsels patients and promotes medication education and adherence.
• Completes regulatory training and maintains documentation per laws and SOPs.
• Supervises interns, technicians, and support staff; provides clinical consultation.
