Sentara Norfolk General Hospital is seeking an Inpatient Case Manager RN for a full-time, 40-hour per week role in Norfolk, VA. The position focuses on coordinating comprehensive care management and patient transitions with the healthcare team.

The ideal candidate will have RN licensure, 12 months of nursing experience, and BLS. BSN preferred and relevant case management certifications are required within one year of eligibility.

Qualifications

• 12 months nursing experience.
• Registered Nurse licensure required.
• Basic Life Support within 90 days.
• BSN preferred.
• Case Management certifications required within one year of eligibility.
• 1 year acute care case management preferred.

Responsibilities

• Provide comprehensive care management and facilitate patient transitions.
• Collaborate with the healthcare team to manage care and transitions.
• Develop and implement a care plan addressing physical, mental, and psychosocial needs.
• Support inpatients identified for comprehensive case management.
• Promote appropriate level of care and LOS through clinical reviews.

Overview:

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager RN to work full-time 40 hours per week.

The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only

Requirements:

• 12 months nursing experience
• Registered Nurse
• Basic Life Support (within 90 days)
• BSN-preferred
• Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility
• 1 year acute care case management preferred
