White Oak Medical Center, part of Adventist HealthCare, seeks an experienced Inpatient Care Navigator/Complex Care Navigator (RN or SW) to join the Care Navigation team. You will assess patients' psychosocial needs and coordinate discharge planning to ensure timely transitions.

The RN or SW collaborates with physicians, nurses, social workers and Utilization Review, identifies barriers to discharge, connects patients with community resources, and participates in interdisciplinary rounds.

White Oak Medical CenterIf you are a current Adventist HealthCare employee, please click this link to apply through your Workday account.White Oak Medical Center seeks to hire an experienced Inpatient Care Navigator/Complex Care Navigator RN or SW for our Care Navigation department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.As a Care Navigator/Complex Care Navigatorr RN or SW you will:• Complete comprehensive psycho-social assessment and review of clinical needs of patients with focus on patient’s requirements as they transition to the next level of care.• Collaborate with physicians, nurses, social workers and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning.• Identify and navigate patient testing and treatment to reduce barriers to patient discharge and preventing delays in patient care; communicates barriers to leadership for resolution and trending• Communicate with Utilization Review staff on any denials, issues or barriers to discharge.• Identify services and resources available in the community and assists with patient connection to these services,• Participate in Interdisciplinary Rounds and other patient care conferences• Documents assessments and interventions according to departmental standards.• Communicate with Utilization Review staff on any denials, issues or barriers to discharge• Participate in process improvement activitiesQualifications include:Registered Nurse (RN)• Graduate of an accredited school of nursing with a minimum of an associate of science degree required, bachelor’s degree preferred• RN-minimum of 1-3 years clinical experience required• Licensed as Registered Nurse with Maryland Board of Nursing• 3-5 years of inpatient hospital case management experience preferred• Vast experience in discharge planning preferred• Local resources experience preferredSocial Worker (SW)• Bachelor of Social Work (BSW) or Master of Social Work (MSW)• Minimum of 3 years of experience in diverse clinical setting is required• Prior case management or discharge planning experience is preferred• Prior experience with Cerner EMR is preferred• LBSW, LMSW, LCSW• BLS required• 3-5 years of inpatient hospital case management experience preferred• Vast experience in discharge planning preferred• Local resources experience preferredWork Schedule:Employment Type: Variable Part-timeHours per Week: 4 - 8 hours/weekTypical Daily Schedule: 1 shift a week as neededShift Type: Rotating Day Shift-Weekends and HolidaysHoliday Requirements: Follow company holiday calendarShift Differentials: YesOrientation is expected to be approximately 3–4 weeks and can be flexible ( one week on, one week off).Pay Range:$77,313.60 - $115,980.80If the salary range is not displayed or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.Under the Fair Labor Standards Act (FLSA), this position is classified as:United States of America (Exempt)At Adventist HealthCare our job is to care for you.We do this by offering:Work life balance through nonrotating shiftsRecognition and rewards for professional expertise403(b) retirement planFree Employee parkingEmployee Assistance Program (EAP) supportAs a faith-based organization, with over a century of caring for the communities in the Maryland area, Adventist HealthCare has earned a reputation for high-quality, compassionate care.
