Job summary

CentraState Healthcare System in Freehold, NJ, is seeking an Inpatient Care Manager to coordinate clinical and psychosocial care, ensuring safe transitions between care levels. The role emphasizes comprehensive assessment, planning, and collaboration with the interdisciplinary team to optimize outcomes and resource use.

The position requires a BSN, NJ RN license, and at least 3 years of acute care experience. Case management training and certification are preferred.

Qualifications

• Bachelors of Nursing required.
• Minimum 3 years acute care RN experience.
• Prior clinical experience in care and management of hospitalized patients.
• Case management training from a professional Case Management organization, preferred.
• RN license required/NJ.
• Case Management certification preferred.
• Excellent communication, negotiation, and conflict resolution skills.
• Data and computer skills.
• Knowledge of relevant and state regulatory standards related to Case Management.
• Rapid cycle change or clinical performance improvement expertise.

Responsibilities

• Develops Case Management Plan: Reviews clinical findings and diagnostic reports to maintain a comprehensive understanding of the patient’s current plan of care.
• Refers High Risk Patients to Social Services: identifies high‑risk patients and refers to social worker.
• Implements Actions to Prevent Readmissions: conducts patient assessments and intervenes to prevent readmissions.
• Develops and Implements Transition of Care Plan: documents discharge/transition plan and coordinates activities.
• Monitors and Escalates Financial Management Issues: communicates payer-related issues impacting post‑acute plan.
• Facilitates Patient Progression: monitors LOS and participates in Care Rounds to meet targets.
• Coordinates Transitions of Care: communicates with patients, families, physicians, and care team.
• Maintains knowledge of regulatory rules including CMS, Joint Commission, Medicare.
• Provides education and resources to patient/family and health care team.
• Identifies potential risk/quality issues and escalates accordingly.
• Other duties as assigned by management.
