BrightStar Care Concord, Lexington & Woburn is seeking Registered Nurses (RN) to join our rockstar Clinical Team in home health. You will manage Plan of Care, supervise staff, ensure clinical compliance, and support infusion-related care across multiple MA locations.

Qualifications

• Active RN license in the state of operation.
• 2–3 years of home health, hospice, or skilled nursing experience.
• Proficient in IV infusion therapy administration.
• CPR certification required.
• Flexibility and on-call availability.

Responsibilities

• Develop and implement patient care plans with physicians and other providers.
• Conduct in-home assessments and ensure regulatory compliance.
• Oversee staff performance and supervision in home care.
• Manage scheduling and staffing coverage for cases.
• Ensure Plan of Care accuracy and clinical documentation.
