Responsibilities

• Provide direct nursing services and case management related to infusion therapy in home, clinic, or alternate living site.
• Collaborate with physicians, payor, patient and caregivers, and other providers to coordinate care.
• Ensure compliance with licensing, safety and documentation standards across Oregon and SW Washington.

Job description

Provides direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. This is accomplished through collaboration and coordination with various providers, the patient and/or caregiver, the physician and the payor. These services are provided in the states of Oregon and/or southwest Washington.

Required Qualifications:
