Job summary

Kaiser Permanente seeks an Outpatient Infusion Pharmacist to deliver comprehensive clinical and distributive pharmaceutical services, including sterile and non-sterile compounding and pharmacokinetic dosing, across home care and outpatient settings.

The role includes monitoring IV therapies, collaborating with the health care team, performing medication reconciliation, and educating patients and caregivers on therapy, safety, and adherence.

Qualifications

• Minimum 1500 hours acute care or infusion pharmacy experience in last 3 years, or completed residency within 5 years.
• Degree in Pharmacy meeting licensure requirements.

Responsibilities

• Provide comprehensive clinical and distributive pharmaceutical services.
• Compounding (sterile and non-sterile) and pharmacokinetic dosing.
• Monitor IV medication therapy in home care, LTC, hospice, and outpatient settings.
• Collaborate with care team to optimize patient outcomes.
• Perform medication reconciliation and ensure formulary adherence.
• Educate patients and caregivers on medication usage and safety.
