Qualifications

• Commitment to regulatory standards, infection control, and continuous improvement.
• Doctor of Pharmacy (PharmD) or equivalent.
• Active/permitted WA pharmacist license.

Responsibilities

• Verify orders and assess dosing and compatibility.
• Prepare sterile admixtures per USP/state standards.
• Collaborate with prescribers and care team to plan therapy.
• Counsel patients and support transitions between care sites.
• Maintain documentation and manage inventory and cold-chain storage.
• Ensure regulatory and accreditation compliance.
• Participate in quality improvement for patient safety.
• Communicate with patients and caregivers about therapy and follow-up.
