Job summary

Soleo Health is seeking a Part-time Infusion Nurse to support infusion patients in Windsor, CO and nearby areas. This role provides primary patient care in home or alternate sites, including assessments, care planning, education, and care coordination with the broader care team.

Ideal candidates hold an active RN license in Colorado, with 3+ years nursing experience and infusion therapy exposure. BSN preferred; Port access and IV skills highly valued, and on-call availability may be required.

Qualifications

• Current RN license in good standing with the State of Colorado Board of Nursing.
• BSN preferred from an accredited nursing program.
• Minimum of 3 years of clinical nursing experience, infusion therapy preferred.
• Demonstrated expertise in peripheral IV insertion (90%+ success rate preferred).
• Accessing and de-accessing implanted ports (PACs/Port-a-Caths) comfortable.
• Experience with infusion therapies including biologics, immunoglobulins, specialty meds, IV hydration.
• Advanced certifications (CRNI, ONS, IgRN) preferred but not required.
• ICU/CCU/ED/Oncology/Vascular/Cardiac care experience preferred.
• Strong clinical assessment, critical thinking, and patient education skills.
• Administrative oversight, workflow monitoring, policy compliance.
• Emergency availability for nights/weekends/holidays.
• Proficient with Outlook and Office; experience with AlayaCare, CPR+, Insight, Concur, Paylocity preferred.
• Excellent communication, organizational, and customer service; able to work independently.

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation to ensure continuous nursing support services to our customers.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

Schedule:

• Part-time Position: 30-39 hours per week, flexible schedule.
• Must be knowledgeable of IV access devices to include: IV lines, ports, and administration of Infusion medications.
• Must be comfortable providing infusion therapy and patient education in home-based settings.
• Must be able to drive up to 1 hour to see patients with the flexibility to cover distances of up to 2 hours on an as-needed basis (we offer mileage reimbursement and hourly rate of pay- even when driving!)

Requirements

• Current Registered Nurse (RN) license in good standing with the State of Colorado Board of Nursing.
• BSN preferred from an accredited nursing program.
• Minimum of 3 years of clinical nursing experience, with infusion therapy experience strongly preferred.
• Demonstrated expertise in peripheral IV (PIV) insertion, including the ability to successfully establish vascular access on the first attempt in the vast majority of patients (90%+ success rate preferred).
• Must be comfortable with accessing and de-accessing implanted ports (PACs/Port-a-Caths) and troubleshooting common vascular access issues.
• Experience administering a wide range of infusion therapies, including biologics, immunoglobulins, specialty medications, and IV hydration preferred.
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
• ICU, Critical Care, Emergency Department, Oncology, Vascular Access, and/or Cardiac Care experience preferred.
• Strong clinical assessment, critical thinking, and patient education skills.
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives.
• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency.
• Proficient with Microsoft Outlook and the Microsoft Office Suite. Experience with AlayaCare, CPR+, Insight, Concur, and Paylocity is preferred.
• Excellent communication, organizational, and customer service skills with the ability to work independently and collaboratively.
• Valid state-issued driver's license with an acceptable driving record and reliable transportation.
