Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Saginaw, MI area and surrounding communities. The nurse will provide direct patient care, assessments, plans of care, and education in home or alternate sites, following our care matrices and standards.

Responsibilities include coordinating with the care team, documenting encounters, and maintaining proficiency with infusion therapy protocols.

Qualifications

• Current RN license by Michigan Board of Nursing in good standing.
• RN degree; BSN from an accredited institution preferred.
• Advanced certification in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required.
• Administrative oversight: monitor workflows, ensure policy compliance and efficient resource use.
• Emergency availability: respond promptly during emergencies, including nights/weekends/holidays.
• Minimum 3 years nursing experience with infusion therapy preferred.
• ICU and/or Cardiac Care experience preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.
• Valid state issued driver’s license with no significant restrictions.
• Single state RN license for Michigan in good standing.

• Document all patient encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment and required clinical documents per policy and state guidelines.
• Collaborate with departments to ensure all patient documents are completed and returned timely.

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusio therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

Schedule:

• Must be able to work Per Diem, 20-24 hours per week, Monday-Friday 8:30am-5pm- variable schedule to include: days/afternoons/evening/weekends as needed
• Must be able to participate in an on-call rotation
• Must be knowledgeable and have used the following IV access devices: IV lines, accessing ports, and administration of infusion medications
• Must be able to drive up to 2 hours one direction to see patients (we offer mileage reimbursement and hourly rate of pay- evening when driving!)
• Must be able to provide infusions to patients in their homes
• Must be comfortable working in patients homes around animals/cigarette smoke
• Must have a single state RN license for Michigan in good standing
• Must live within 25 miles from the Saginaw, MI area

Requirements

• Current RN license by the applicable state’s Board of Nursing, in good standing
• RN degree, bachelor’s degree in nursing (BSN) from an accredited institution preferred
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives
• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency
• Minimum 3 years nursing experience with background in infusion therapy preferred
• Experience in ICU and/or Cardiac Care preferred
• Proficient with Microsoft Outlook and Microsoft Office Suite
• Valid state issued driver’s license with no significant restrictions on that license
