Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to provide primary patient care for infusion patients in the home or alternate sites. You will perform assessments, care planning, education, and evaluation within established care matrices and safety standards.

Requirements include an active Colorado RN license, 3+ years in nursing with infusion therapy experience, and the ability to drive to patient homes. Schedule is 10–20 hours per week, with days, evenings or weekends as needed.

Qualifications

• Current RN license by the applicable state’s Board of Nursing; State of Colorado
• RN degree, BSN from an accredited institution preferred
• Advanced certification in specialty areas (CRNI, ONS, IgRN) preferred but not required
• Administrative Oversight: responsible for workflows, policies, resources
• Emergency Availability: available during non-standard hours
• Minimum 3 years nursing experience with infusion therapy background preferred
• Experience in ICU and/or Cardiac Care preferred
• Proficient with MS Outlook and Microsoft Office Suite; CPR+, AlayaCare, Insight, Concur, Paylocity helpful
• Valid state driver's license with no significant restrictions

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with care team members
• Evaluate and educate patient/care partner to safely administer home infusion therapy
• Evaluate home environment and identify safety interventions
• Develop plan of care with team and evaluate response to therapy
• Manage schedule via company platform and communicate changes
• Coordinate patient care via approved platform
• Participate in nurse on-call rotation for continuous nursing support
• Document all patient encounters and care activities at time of service
• Maintain knowledge of Soleo therapies, protocols, and standards
• Ensure timely submission of POTs and required documents per policy
• Ensure required patient documents are completed and returned to branch office
• Perform other duties as assigned

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation to ensure continuous nursing support services to our customers.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.
• Perform other duties as assigned.

Schedule:

• Ability to work Monday-Friday 8:30am-5pm- variable schedule to include: days, afternoons, evenings, and weekends as needed- Per Diem- 10-20 hours per week
• Must have a minimum of 2-3 days availability to see patients during the week
• Must be knowledgeable and have used the following IV access devices to include: IV lines, accessing ports, and administration of Infusion medications (to include PIV skills, central lines, PICCS, troubleshooting infusion pumps)
• Must be able to drive up to 1 hour to see patients in their homes (we offer mileage reimbursement and hourly rate of pay- even when driving!)
• Must be comfortable administering infusions to patients in their homes
• Must be licensed as an RN in Colorado - in good standing

Requirements

• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives
• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency
• Minimum 3 years nursing experience with background in infusion therapy preferred
• Experience in ICU and/or Cardiac Care preferred
• Proficient with Microsoft Outlook and Microsoft Office Suite.Additional computer knowledge of the following programs: AlayaCare, Insight, CPR+, Concur, and Paylocity helpful
• Valid state issued driver’s license with no significant restrictions on that license
