Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Detroit, MI area. The role requires an active Michigan RN license, 3+ years in infusion or related areas, and a willingness to drive to patient homes. Per diem schedule, with mileage reimbursement and on-call duties.

You will administer and monitor home infusion therapy, coordinate with the care team, and educate patients and families while complying with policies and state guidelines.

Qualifications

• Current RN license in Michigan in good standing.
• BSN preferred, not required.
• Advanced certifications (CRNI, ONS, IgRN) preferred.
• Experience with home infusion or infusion therapy is valued.
• Willingness to participate in on-call rotations and drive to patient homes.

Responsibilities

• Provide direct infusion nursing care in patients' homes or alternate sites.
• Coordinate care with care team, including pharmacists and prescribers.
• Educate patient and care partner on infusion therapy administration.
• Assess home environment for safety and suitability of care.
• Develop and evaluate care plans with the team; document encounters.
• Manage schedule and on-call rotations; travel as needed; ensure regulatory compliance.

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting.

Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.


• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.


• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.


• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.


• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.


• Coordinate patient care via approved company platform.


• Participate in the nurse on-call rotation to ensure continuous nursing support services to our customers.


• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.


• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.


• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.


• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

Schedule:

• Must be able to work Per Diem, 20-24 hours week, 3 days per week - variable schedule to include: days, afternoons, evenings, and weekends as needed


• Must be knowledgeable of IV access devices to include: IV lines, accessing ports, and administration of infusion medications


• Must be able to participate in an on-call rotation


• Must be able to drive up to 1.5 hours one direction to see patients as needed (we offer mileage reimbursement and hourly rate of pay- even when driving!)


• Must be comfortable working in patients homes around animals/cigarette smoke


• Must be comfortable administering infusions to patients in their homes


• Must hold a single state RN license for the state of Michigan in goodstanding


• Must live within 25 minutes from the Detroit, MI area

Requirements

• Current RN license by the applicable state's Board of Nursing, in good standing - state of Michigan


• RN degree, bachelor's degree in nursing (BSN) from an accredited institution preferred


• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required


• Participation in an on-call rotation, nights and weekends included, providing as-needed support outside of regular business hours to address urgent issues and ensure continuous operations


• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives


• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency


• Minimum 3 years nursing experience with background in infusion therapy preferred


• Experience in ICU and/or Cardiac Care preferred


• Proficient with Microsoft Outlook and Microsoft Office Suite


• Valid state issued driver’s license with no significant restrictions on that license
