Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite under a physician's plan of care. The role emphasizes patient safety, documentation, and collaboration with the clinical team.

The ideal candidate will hold an active RN license, have at least 1 year of nursing experience, and possess CPR certification. This position offers growth through training, mentorship, and a mission-driven culture.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 years of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit and documents findings.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Communicates with clinical teams and physicians to ensure continuity of care and timely documentation.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
