Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Findlay, OH and surrounding areas. You will provide direct patient care, educate families, and coordinate care with the team in home or alternate settings.

Requirements include an active RN license in Ohio, BSN preferred, infusion experience (ICU/cardiac a plus), and availability for on-call shifts. Valid driver’s license and ability to travel within 25 minutes of Findlay are needed.

Qualifications

• Current RN license by the state Board of Nursing in good standing.
• BSN preferred; RN degree from an accredited program.
• Minimum 3 years nursing experience with infusion therapy preferred.
• ICU and/or Cardiac Care experience preferred.
• On-call rotation including nights and weekends.
• Valid driver’s license with no major restrictions.
• Residence within 25 minutes of Findlay, OH.

Responsibilities

• Provide direct care for assigned patients and coordinate with care team.
• Educate patient/care partner for home infusion therapy and safety.
• Assess home environment and implement safe care measures.
• Develop and evaluate the care plan with the team.
• Manage schedule and coordinate patient care via approved platform.
• Participate in nurse on-call rotation to support services.

• Maintain knowledge of Soleo therapies, protocols, and standards.
• Ensure timely POTs and required clinical documentation.

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation to ensure continuous nursing support services to our customers.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

Schedule:

• Monday-Friday 8:30am-5pm variable schedule to include days, afternoons, evenings, and weekends as needed - Per Diem 12-16 hours per week
• Must be able to participate in an on-call rotation to include 1 weekend every 2-3 months
• Must be able to drive your own properly registered/insured vehicle up to 1.5 hours one direction to see patients as needed (we offer mileage reimbursement and hourly rate of pay - even when driving!)
• Must be able to work in patient homes around animals/cigarette smoke
• Must be knowledgeable and have used the following IV access devices to include: IV lines, accessing ports, and administration of infusion medications
• Must have a single state RN license in good standing for the state of Ohio
• Must live within 25 minutes from the Findlay, OH area

Requirements

• Current RN license by the applicable state’s Board of Nursing, in good standing. Single State RN License for Ohio
• RN degree, bachelor’s degree in nursing (BSN) from an accredited institution preferred
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
• Participation in an on-call rotation, nights and weekends included, providing as-needed support outside of regular business hours to address urgent issues and ensure continuous operations.
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives.
• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency.
• Minimum 3 years nursing experience with background in infusion therapy preferred.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.
• Valid state issued driver’s license with no significant restrictions on that license.
