Job description

Position SummaryProvides skilled nursing and infusion therapy services within the LPN scope of practice, under the direction and supervision required by Washington State law. Provides patient care in accordance with the established plan of care, provider orders, clinical standards, and Agency policies and procedures. Recognizes and reports changes in patient condition and maintains safe, compliant, patient-centered care.Essential Job Functions• Provides skilled nursing and infusion therapy care within the LPN scope of practice and established plan of care.• Administers IV medications, fluids, and assigned infusion therapies, including antibiotics, TPN, immunoglobulin, biologics, chemotherapy, and other specialty medications, as permitted by law, provider orders, training, competency, and Agency policy.• Performs venipuncture, peripheral IV insertion, vascular access care and access, dressing changes, specimen collection, and infusion pump management within demonstrated competency, authorized provider orders, Agency policy, and applicable regulatory requirements.• Observes and monitors patients, infusion sites, and vascular access devices for adverse reactions, complications, and changes in condition; initiates emergency procedures as indicated by Agency policy; and promptly reports concerns to the supervising RN and/or authorized provider.• Follows medication-safety, infection-control, aseptic technique, and patient-safety standards.• Reinforces the established patient/caregiver teaching plan and provides routine patient and caregiver education regarding medications, infusion therapy, treatment procedures, and ongoing care within the established plan of care.• Collaborates with RNs, physicians, pharmacists, and other members of the interdisciplinary team to support safe and effective patient care.• Maintains accurate, timely documentation of care, treatments, patient response, education, and communication in accordance with Agency and regulatory requirements.• Maintains competency in assigned infusion therapies, vascular access procedures, and related equipment and participates in required training and competency validation.• Performs other duties as assigned within the LPN's legal and individual scope of practice, education, training, and demonstrated competency.Duties, responsibilities, and activities may change at any time with or without notice to meet organizational needs.Supervisory Responsibilities• There are no supervisory responsibilities for this role.• Provides nursing care under authorized provider orders and under the direction and supervision of a Registered Nurse in accordance with chapter 18.79 RCW, chapter 246-840 WAC, applicable in-home services regulations, and Agency policy. The supervising RN need not be physically present when permitted by law and Agency policy.• Reports to Nurse Manager and Director of Clinical Services.• May provide orientation, peer support, or task-specific assistance to other staff within the LPN’s scope of practice as permitted by law and agency policy.Required Education, Licensure & ExperienceAll requirements below are job-related and consistent with business necessity.• Current, unrestricted Washington State Licensed Practical Nurse (LPN) license.• Current BLS/BCLS certification.• Infusion therapy experience preferred; demonstrated competency in assigned infusion procedures required.• Experience in acute care, home health, outpatient, community health, or infusion therapy preferred.• Current TB testing as required by agency policy.• Valid Washington State driver's license, reliable automobile, and current automobile insurance.• Ability to communicate effectively in English, verbally and in writing.• Basic computer/EHR proficiency.• Ability to practice within the Washington State Nurse Practice Act and applicable regulations.Preferred Education & ExperiencePreferred qualifications are not required but are job-related and consistent with business necessity.• Home infusion or specialty pharmacy experience preferred.• IV therapy/infusion training or certification preferred.• Additional languages preferred.Core Competencies• Provides safe, competent infusion therapy within LPN scope of practice.• Recognizes and promptly reports changes in patient condition.• Demonstrates competency with vascular access devices and infusion equipment.• Communicates effectively with patients, caregivers, RNs, providers, and pharmacists.• Maintains accurate documentation and patient confidentiality.• Demonstrates sound judgment within the LPN scope of practice.Physical DemandsFor physical demands of position, including vision, hearing, repetitive motion and environment, see the following description. The position may involve exposure to outdoor weather conditions, medications, chemicals, fumes, and airborne particles.
