Registered Nurse
Position Summary

The Registered Nurse (RN) is responsible for providing safe, high-quality, patient-centered nursing care in an outpatient/community healthcare setting. The RN performs clinical assessments, triage, patient intake, medication administration, care coordination, patient education, and follow-up activities.

The RN works collaboratively with providers, clinical staff, and internal and external partners to promote positive health outcomes, ensure timely access to care, and support continuity of services. The position also contributes to quality improvement initiatives, regulatory compliance, and maintaining a safe and effective clinical environment.

Key Responsibilities
  • Provide direct patient care and maintain accurate, clear, and timely clinical documentation.
  • Perform in-person and telephone triage, assess patient symptoms, prioritize care needs, and elevate situations requiring additional clinical attention.
  • Complete nursing intake processes for new patients and support timely access to healthcare services.
  • Administer medications in accordance with physician orders, applicable guidelines, and organizational procedures.
  • Schedule appointments, process medication refills according to established protocols, communicate laboratory results as directed, and monitor referral status.
  • Collaborate with healthcare providers and other internal and external partners to improve patient outcomes and support transitions of care.
  • Identify patient education needs and provide appropriate health education based on individual needs, health literacy, and evidence-based practices.
  • Complete required public health and communicable disease reporting in accordance with applicable regulations.
  • Maintain compliance with applicable healthcare regulations, policies, procedures, and professional nursing standards.
  • Establish trusting relationships with patients and promote continued engagement in care through empathetic, patient-centered interactions.
  • Apply trauma-informed principles when interacting with patients, families, and team members.
  • Provide clinical support and professional guidance to Medical Assistants and elevate concerns to nursing leadership when appropriate.
  • Maintain medication inventory, monitor expiration dates, and assist with maintaining appropriate inventory levels.
  • Participate in staff development and continuing education to remain current with treatment options, clinical resources, and professional guidelines.
  • Participate in continuous quality improvement initiatives and activities.
  • Support patients with chronic and infectious conditions throughout the continuum of care.
  • Maintain flexibility to provide coverage at different clinic locations and work occasional evenings, weekends, or emergency shifts based on operational needs.
  • Follow all applicable safety, privacy, compliance, and professional practice requirements.
  • Perform other related duties as assigned.
Required Qualifications
Education
  • High School Diploma or GED.
  • Associate Degree in Nursing (ADN).
Preferred Education
  • Bachelor of Science in Nursing (BSN).
Licensure & Certifications
  • Active Registered Nurse (RN) license applicable to the work location.
  • Basic Life Support (BLS) certification from the American Heart Association.
Preferred Experience
  • Approximately 2 years of Registered Nurse experience.
  • Experience in outpatient or community healthcare settings.
  • Knowledge of infectious disease transmission and prevention, including HIV and sexually transmitted infections.
  • Experience providing care to patients with mental health conditions and/or substance use disorders.
Skills & Competencies
  • Strong clinical assessment, triage, critical thinking, and decision-making skills.
  • Excellent verbal and written communication skills.
  • Ability to read, understand, document, and communicate medical information accurately.
  • Strong interpersonal skills and the ability to provide compassionate and empathetic patient care.
  • Ability to work effectively as part of a multidisciplinary healthcare team.
  • Strong organizational skills and attention to detail.
  • Ability to maintain patient confidentiality and comply with privacy requirements.
  • Proficiency with Microsoft Office applications and standard clinical computer systems.
  • Bilingual communication skills may be required depending on the assignment.
Working Conditions & Physical Requirements
  • Ability to stand, walk, twist, and move throughout the workday.
  • Occasional lifting, pushing, or assisting with patient transfers.
  • May encounter clinical environments involving illness, injuries, or other potentially unpleasant conditions.
  • May require evening, weekend, or emergency coverage.
  • May require travel between assigned healthcare locations.
