Job summary

Aviata Health Group is seeking an In-House Nurse Practitioner specializing in Infectious Disease to lead clinical care in skilled nursing facilities. You will evaluate residents, prescribe treatments, and collaborate with the interdisciplinary team to improve outcomes.

This role offers flexible full-time schedules, no nights or weekends, and a strong benefits package including Health, 401(k), PTO, CME support, and licensure/reimbursement programs, with monthly bonuses based on performance.

Qualifications

• Active Florida NP license (unencumbered).
• Master’s Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience preferred, in skilled nursing/long-term care settings.
• Ability to engage and communicate with elderly and chronically ill patients and families.
• Minimum of 12 patient visits per day; bonuses for surpassing target.
• Knowledge of Medicare/Medicaid and Health Plan benefits is a plus.

Responsibilities

• Perform comprehensive physical examinations and evaluate changes in patient conditions.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely communication and documentation of assessments, treatment plans, and follow-ups.
• Support the physician-patient relationship and address gaps in care.
