Qualifications

• Active California PT license required.
• 1 year of licensed Physical Therapist experience preferred.
• Reliable transportation and driver's license required.
• Current CPR certification and professional liability insurance.
• Recent physical examination within 2 years.
• TB test within last 1–2 years depending on test type.
• EIN with active business license.

Responsibilities

• Provide 45 minutes of rehabilitation services in patients' homes.
