Job summary

You will autonomously set your practice days, hours, and fee schedule while maintaining control over patient appointments and billing. Optional equipment and support services may be available where state law allows.

Qualifications

• Doctor of Optometry OD from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.

Responsibilities

• Provide eye and vision care as an independent lease holder within Walmart stores.
• Establish your days and hours of operation and manage your patient schedule.
• Set your fee schedule and keep 100% of exam/professional fees.
• Opt into equipment instrumentation and optional support services where allowed.

As an independent lease holder you have the autonomy to:

• Establish your hours of operation
• Keep 100 of exam/professional fees

For a fair market value you can opt into

• Equipment instrumentation
• Administrative and/or Opt-Tech support services (if state allows)

Minimum Qualifications for a lease agreement:

• Doctor of Optometry OD from an accredited optometry school
• Valid state optometry license
• Ophthalmologist MD or DO from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
