Job summary
Walmart is seeking an independent Optometrist leasing space inside our Walmart stores in Eden Prairie, MN to lead eye and vision care as well as general health.
You will establish your days and hours, manage your patient schedule, and set your fee schedule, keeping 100% of exam/professional fees. Qualified physicians must hold a Doctor of Optometry and a valid state optometry license, with liability certificates as required.
Qualifications
- OD from an accredited optometry school.
- Valid state optometry license.
- Ophthalmologist MD or DO from an accredited school and valid state ophthalmology license (if applicable).
- General Liability Certificate
- Professional Liability Insurance Certificate

- Establish your days of practice and hours of operation.
- Lead patient care in eye and vision health.
- Set your fee schedule and keep 100% of exam fees.
- Negotiate equipment instrumentation and support services if state allows.

As an independent lease holder you have the autonomy to:
- Establish your days of practice
- Establish your hours of operation
- Keep 100 of exam/professional fees
For a fair market value you can opt into
- Equipment instrumentation
- Administrative and/or Opt-Tech support services (if state allows)
Minimum Qualifications for a lease agreement:
- Doctor of Optometry OD from an accredited optometry school
- Valid state optometry license
- Ophthalmologist MD or DO from an accredited school
- Valid state ophthalmology license
- General Liability Certificate
- Professional Liability Insurance Certificate
