Independent Optometrist role inside Sam’s Club at Fort Walton Beach, FL. Launch your services, establish your days and hours, and control your patient schedule while setting your fee schedule. You will keep 100% of exam and professional fees as you build a patient base.

Leasing space includes equipment/instrumentation support (where allowed) and requires OD degree, active state licenses, and standard professional certificates. This is not traditional Walmart employment.

Qualifications

• OD degree from an accredited optometry school.
• Current valid state optometry license.
• Ophthalmology license (MD/DO) from an accredited school.
• Current valid state ophthalmology license where required.
• General liability certificate.
• Professional liability insurance certificate.
• W-9 tax form submission.

Responsibilities

• Establish your days of practice inside Sam's Club.
• Establish hours of operation and control patient schedule.
• Set your fee schedule and keep 100% of exam fees.

Launch your services in Sam’s Clubs! As an Independent Optometrist leasing space inside our Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being.

• Establish your days of practice
• Establish your hours of operation
• Control your patient schedule
• Set your fee schedule
• Keep 100% of exam/professional fees

• Equipment/instrumentation
• Administrative and/or Opt-Tech support services (if state allows)

Minimum Qualifications for a lease agreement:

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9
