Job summary

Firefly Children & Family Alliance is hiring an Independent Living Case Manager. This home-based role operates primarily from Indianapolis, IN, serving clients across Marion, Hamilton, and Hancock counties.

You will collaborate with internal teams and community agencies to advocate for at-risk children and families, conduct needs assessments, and monitor progress while maintaining a flexible schedule that may include evenings and weekends.

Qualifications

• Bachelor's degree in social work or related field is required.
• Must be at least 21 years old.
• In-person work; no remote work available for this position.
• Valid driver's license with acceptable driving record (no 4+ points).
• Knowledge of Indiana social service systems and confidentiality practices.
• Lived or working experience with Foster Care is preferred, not required.

Responsibilities

• Direct Service: provide direct/indirect contact and brokered services to clients.
• Visit and assist clients in achieving goals and provide transportation as needed.
• Attend court with clients and respond to crises when referred.
• Develop relationships with community agencies and collaborate with treatment teams.
• Report writing and information entry, including monthly reports and required documentation.
• Prepare written reports for the Department of Child Services/Probation within deadlines.

Firefly Children & Family Alliance is currently seeking an Independent Living Case Manager! While this position is home-based, the primary office is in Indianapolis, IN. In this position, you will work with clients in Marion, Hamilton, and Hancock counties . You will collaborate with our internal treatment team and community agencies to directly advocate for at-risk children and families, providing important needs assessments, prevention and intervention services, and progress monitoring. This role requires the ability to adapt to a flexible work week, including evenings and potentially weekends.

You Will:

Direct Service – Direct contact, indirect contact, and brokered services (50%)

• Visit and assist each client in achieving their goals


• Broker support to clients


• Provide transportation to clients associated with specific goals


• Attend court with clients


• Respond to crisis (depending upon referral type)


• Develop relationships with agencies in the community


• Work with other social welfare agencies


• Refer clients to community agencies

Report writing, information entry (50%)

• Email referring agent and supervisor following each contact or attempted contact


• Review and upload monthly reports to Kidtraks and NYTD


• Maintain case and agency paperwork according to standards


• Prepare written reports for the Department of Child Services / Probation office within identified time frames

You Have:

• A bachelor's degree in social work or related field is required.


• You must be at least 21 years of age


• Work in-person, no remote work available for this position.


• A valid driver's license and insurance. Must not have 4 or more points on driving record.


• Knowledgeable regarding social service systems within the state


• Knowledge of and adherence to current confidentiality practices


• Appreciation for and ability to respond to cultural differences


• Maintain relevant networks of main contacts


• At least 1 year of home-based case management experience working with clients in Foster Care or transitioning out of Foster Care


• Lived experience in Foster Care is preferred, not required


• Ability to work a flexible schedule, including evenings and some weekends.
