Job summary

EmberHope seeks an Independent Living Aftercare Support Worker in Wichita, KS. You will provide trauma-informed aftercare for young adults, including monthly contact up to six months post-release from DCF custody, and collaborate with the Permanency Services Team to coordinate services.

Responsibilities include completing required documentation within 48 hours of direct service, transporting clients as needed, and supporting families and staff while upholding the company’s guidelines and mission.

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred.
• Experience in Child Welfare or Human Services preferred.
• Must be 21+ and pass background checks and screenings.
• Must be able to travel and lift up to 50 lbs; evenings/weekends may be required.

To be a successful Independent Living Aftercare Support Worker you must be able to do the following always:

• Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
• Develop and maintain Aftercare Contact Agreements that outline the young adult’s responsibilities, needed services, supports, and follow-up expectations.
• Provide aftercare support for young adults for up to six months after release from DCF custody, including monthly contact and support based on the Aftercare Contact Agreement.
• Complete required documentation and reporting, including monthly contact attempts, service updates, guardian contacts when applicable, and submission of the monthly DCF report to supervisor
• Document 100% of direct services provided to the youth 48 hours of the occurrence.
• Transport clients as needed. Travel in state to assure client contact as needed.
• Collaborate with Permanency Specialist, Permanency Services Team, internal, and external stakeholders, demonstrating problem-solving/facilitation skills. Participate in individual and Permanency Services Unit/Dyad supervision once a month or as needed with the supervisor.
• Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.

Requirements

To qualify for this job, you need to have:

• High School Diploma or GED required. Bachelor’s degree in social worker or related field preferred.
• Work Experience - Prior experience working in Child Welfare, Human Services, or related field.
• 21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
• Maintain company-required training.
• Physical working requirements - Ability to travel, driving company or personal vehicle including at night. Ability to do occasional lifting of up to 50 lbs. Able to walk stairs. Ability to work flexible schedule, including some evenings and weekends. Employees are subject to outside environmental conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change.
